Preadmission beta-blocker use and 30-day mortality among patients in intensive care: a cohort study.

ABSTRACT: INTRODUCTION: Beta-blockers have cardioprotective, metabolic and immunomodulating effects that may be beneficial to patients in intensive care. We examined the association between preadmission beta-blocker use and 30-day mortality following intensive care. METHODS: We identified 8,087 patients over age 45 admitted to one of three multidisciplinary intensive care units (ICUs) between 1999 and 2005. Data on use of beta-blockers and medications, diagnosis, comorbidities, surgery, markers of socioeconomic status, laboratory tests upon ICU admission, and complete follow-up for mortality were obtained from medical databases. We computed probability of death within 30-days following ICU admission for beta-blocker users and non-users, and the odds ratio (OR) of death as a measure of relative risk using conditional logistic regression and also did a propensity score-matched analysis. RESULTS: Inclusion of all 8,087 ICU patients in a logistic regression analysis yielded an adjusted OR of 0.82 (95% CI: 0.71 to 0.94) for beta-blocker users compared with non-users. In the propensity score-matched analysis we matched all 1,556 beta-blocker users (19.2% of the entire cohort) with 1,556 non-users; the 30-day mortality was 25.7% among beta-blocker users and 31.4% among non-users [OR 0.74 (95% CI: 0.63 to 0.87)]. The OR was 0.69 (95% CI: 0.54 to 0.88) for surgical ICU patients and 0.71 (95% CI: 0.51 to 0.98) for medical ICU patients. The OR was 0.99 (95% CI: 0.67 to 1.47) among users of non-selective beta-blockers, and 0.70 (95% CI: 0.58 to 0.83) among users of cardioselective beta-blockers. CONCLUSIONS: Preadmission beta-blocker use is associated with reduced mortality following ICU admission.

Multinational, observational study of procalcitonin in ICU patients with pneumonia requiring mechanical ventilation: a multicenter observational study.

ABSTRACT: INTRODUCTION: To determine whether serum procalcitonin (PCT) levels is associated with prognosis, measured as organ dysfunction and 28 day mortality, in patients with severe pneumonia. METHODS: Multicenter, observational study of critically ill adult patients with pneumonia requiring mechanical ventilation conducted in ten academic hospitals in Canada, the United States, and Central Europe. PCT was measured daily for 14 days using an immuno-luminometric assay. RESULTS: We included 175 patients, 57 with community acquired pneumonia (CAP), 61 with ventilator associated pneumonia (VAP) and 57 with hospital acquired pneumonia (HAP). Initial PCT levels were higher in CAP than VAP patients (median [interquartile range]; 2.4 [0.95-15.8] vs. 0.7 [0.3-2.15], ng/ml, P<0.001) but not significantly different to HAP (2.2 [0.4-8.0] ng/ml). 28-day ICU mortality rate for all patients was 18.3% with a median ICU length of stay of 16 days (range 1-142 days). PCT levels were higher in non-survivors than in survivors. Initial and maximum PCT levels correlated with maximum SOFA score r2=0.50 (95% CI: 0.38-0.61) and r2=0.57 ( 0.46-0.66), respectively. Receiver operating curve analysis on discrimination of 28-days mortality showed areas under the curve of 0.74, 0.70, and 0.69 for maximum PCT, initial PCT, and APACHE II score, respectively. The optimal cut-off to predict mortality for initial PCT was 1.1 ng/ml (odds ratio 7.0 [95% CI 2.6-25.2]) and that for maximum PCT was 7.8 ng/ml (odds ratio 5.7 [95% CI 2.5-13.1]). CONCLUSIONS: PCT is associated with the severity of illness in patients with severe pneumonia and appears to be a prognostic marker of morbidity and mortality comparable to the APACHE II score.

Activated protein C in septic shock: a propensity-matched analysis.

ABSTRACT: INTRODUCTION: The use of human recombinant activated protein C (rhAPC) for the treatment of severe sepsis remains controversial despite multiple reported trials. The efficacy of rhAPC remains a matter of dispute. We hypothesized that patients with septic shock who were treated with rhAPC had an improved in-hospital mortality compared to patients with septic shock with similar acuity who did not receive rhAPC. METHODS: This retrospective cohort study was completed at a large University-affiliated hospital. All patients with septic shock admitted to a 50-bed ICU between July 2003 and February 2009 were included. Patients were treated according to sepsis management guidelines. RESULTS: A total of 563 septic shock patients were included (110 received rhAPC and 453 did not). Treated and untreated groups were matched in patient characteristics, comorbidities, and physiologic variables in a 1:1 propensity-matched analysis (108 received rhAPC, 108 did not). Mean Acute Physiology And Chronic Health Evaluation II (APACHE II) scores were 24.5 for the matched treated and 23.9 for the matched untreated group (P=0.54). Receipt of rhAPC was associated with reduced in-hospital mortality (35.2% vs. 53.8%, P=0.005), similar mean days on vasopressors (2 vs. 2, P=0.90), similar mean days on mechanical ventilation (9 vs. 8.7, P=0.80), similar mean length of ICU stay in days (11.0 vs. 11.3, P=0.90), and similar mean length of hospital stay in days (19.5 vs 27, P=0.11). No patients in either group had intracranial bleeding; differences in gastrointestinal bleeding and transfusion requirements were not statistically significant. CONCLUSIONS: Patients in our institution with septic shock who were treated with rhAPC had a reduced in-hospital mortality compared with patients with septic shock with similar acuity who were not treated with rhAPC. In addition, time on mechanical ventilation, time on vasopressors, lengths of stay and bleeding complications did not differ between the groups.

Residual vein obstruction to predict the risk of recurrent venous thromboembolism in patients with deep vein thrombosis: A systematic review and meta-analysis.

Background: Residual vein obstruction (RVO) detected on compression ultrasonography of the leg after a few months on anticoagulation therapy might be able to identify patients with deep vein thrombosis (DVT) at high risk of having a recurrent venous thromboembolism (VTE). Purpose: To determine whether RVO is associated with an increased risk of recurrent events in patients with DVT. Data Source: A systematic literature search strategy was conducted using MEDLINE, EMBASE and the Cochrane Register of Controlled Trials. Study Selection: We selected 14 articles (9 prospective cohort studies and 5 randomized controlled trials) that included patients with DVT who had a assessment for RVO using compression ultrasonography. Data extraction: Two reviewers independently extracted data onto standardized forms. Data Synthesis: Overall, the presence of RVO is not associated with an increased risk of recurrent VTE (OR: 1.24, 95% CI: 0.9 to 1.7) in patients with unprovoked DVT that stopped oral anticoagulation therapy at the time of RVO assessment. However, RVO was significantly associated with recurrent VTE in patients with any (unprovoked and provoked) DVT: OR 1.5 (95% CI: 1.1 to 2.0). Conclusion: Residual vein obstruction was associated with a modest increased risk of recurrent VTE in patients with DVT (unprovoked and provoked). However, RVO does not seem to be a predictor of recurrent VTE in patients with unprovoked DVT following anticoagulation discontinuation. Further prospective studies are needed to assess the role of RVO in patients with unprovoked DVT.

The impact of computerized provider order entry systems on medical-imaging services: a systematic review.

Background Computerized provider order entry (CPOE) systems have been strongly promoted as a means to improve the quality and efficiency of healthcare. Methods This systematic review aimed to assess the evidence of the impact of CPOE on medical-imaging services and patient outcomes. Results Fourteen studies met the inclusion criteria, most of which (10/14) used a pre-/postintervention comparison design. Eight studies demonstrated benefits, such as decreased test utilization, associated with decision-support systems promoting adherence to test ordering guidelines. Three studies evaluating medical-imaging ordering and reporting times showed statistically significant decreases in turnaround times. Conclusions The findings reveal the potential for CPOE to contribute to significant efficiency and effectiveness gains in imaging services. The diversity and scope of the research evidence can be strengthened through increased attention to the circumstances and mechanisms that contribute to the success (or otherwise) of CPOE and its contribution to the enhancement of patient care delivery.

Terlipressin in Hepatorenal Syndrome (March).

OBJECTIVE: To compare the pharmacology, dosing, and adverse reactions of vasopressin and terlipressin for the treatment of hepatorenal syndrome (HRS) and assess the efficacy of the investigational drug terlipressin for HRS. DATA SOURCES: Articles evaluating prospective studies for vasopressin and terlipressin were discussed after being identified through PubMed (1966-November 2010), International Pharmaceutical Abstracts (1970-November 2010), and EMBASE (1985-November 2010) with combinations of the following terms: vasopressin, terlipressin, and hepatorenal syndrome. In addition, reference citations from publications identified were reviewed. Thirteen studies were identified for terlipressin, along with 4 meta-analyses and 1 case report. For vasopressin, 2 studies were identified. STUDY SELECTION AND DATA EXTRACTION: Prospective clinical studies directly comparing terlipressin and vasopressin were evaluated, as well as prospective clinical studies and meta-analyses for terlipressin in HRS. DATA SYNTHESIS: No randomized, placebo-controlled trials using vasopressin for the treatment of type I HRS have been published, and 4 randomized studies involving 197 patients provide the most current outcome data for use of terlipressin in HRS. Terlipressin differs significantly from vasopressin with regard to its pharmacology, dosing, and adverse drug reaction profile. There is a paucity of data on vasopressin for HRS. CONCLUSIONS: No definitive recommendations can be made for the use of terlipressin for this indication until further, well-conducted studies are performed.
